Responsibilities:
The primary function of the Intake Coordinator is to facilitate the referral intake process, coordinate care with the interdisciplinary team, the patient, their family, and the referral source. May be required to perform in-home visits.ential Functions
  • Receives and appropriately enters/documents new referrals in a timely manner.
  • Complete workflow tasks and other assignments to the IC Responsible Position in the Agency’s electronic medical record.
  • Participates in coordinating care with nurses, patients, and the interdisciplinary team.
  • Provides effective communication to patients, staff members, other health care professionals and referral sources.
  • Responds to Agency and patient needs in a professional and creative manner.
  • Demonstrates commitment, professional growth, and competency.
  • Participates in Agency-sponsored in-service training.
  • Participates in the Agency’s QAPI program.
  • Promotes the Agency philosophy and administrative policies to ensure quality of care.
Qualifications:
Education: Graduate of an accredited school of Vocational/Practical nursing preferred. If non-clinical, must have high school diploma or equivalent.

Licensure: Current state license as a Licensed Vocational Nurse or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) preferred. Current driver’s license.

Experience: One year experience as a Licensed Vocational Nurse/Licensed Practical Nurse, home health experience preferred.


Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.


Transportation: Reliable transportation and valid and current auto liability insurance.
