Full Time

Position available for Assessment Nurse to provide medical assessment to elderly and adults with physical disabilities in Forsyth County for the NC Medicaid CAP/DA program. Will develop assessment team review with case managers and maintain documentation to meet program requirements. LPN license required with experience working in geriatrics, home health, long term care or CAP/DA preferred. Excellent communication skills and interpersonal skills working with diverse backgrounds and lifestyles. Good organizational and documentation skills needed.

Education, Experience, Training:

Nurse who holds a current North Carolina LPN license with two (2) year or four (4) year degree. Experience in geriatrics, home health, long term care, or CAP/DA preferred.

Knowledge, Skills, Ability:

• Basic competency in Microsoft Word and Excel
• Understanding and sensitivity to diverse backgrounds and lifestyles
• Knowledge of community resources
• Medical history assessment skills
• Ability to articulate ideas clearly, (both written and orally), listen, and establish personal rapport
• Good organizational and documentation skills
