POSITION SUMMARY
You will be delivering a financial breakdown of the overall Care Plan along with helping patients to coordinate their schedules to arrive for the recommended care.
You will be responsible for acquiring long term, quality patients for the clinic through the implementation of proven lead conversion systems and sales techniques.
As the Case Manager, it will be your responsibility to build and maintain patient relationships throughout the patient’s journey to wellness at Chronic Care of Richmond. You will participate in the educating of patients while working with the clinicians as they report the findings of an orthopedic exam and diagnostic imaging.
You will focus on understanding our patient’s needs and goals while helping them overcome obstacles to receiving care. You will learn to empathize with their struggles while influencing the patient to do what is in their best interest to improve their quality of life.
You will verify insurance coverage for recommended treatment plans and communicate all covered and out of pocket costs to the patients.
You will demonstrate an expert level of knowledge and experience in these areas, as well as actively work and collaborate effectively within a highly team-centric environment.
PERFORMANCE OBJECTIVES
- Create and deliver financial estimates, secure financial commitment, and confirm long-term scheduling to patients based on Provider’s orders
- Develop emotional connection with patients to understand their expectations, goals, and desired outcomes
- Perform initial consultations on new patients to determine medical history, motivating factor that prompted patient to look for help with their problem and functional goals of the patient
- Discover and provide motivating factors that prompted patients to seek help.
- Establish functional goals of the patient and be able to communicate with your team.
- Execute patient follow up calls, lead conversion techniques, and patient retention activities
- Provide and ensure completion of all patient paperwork
- Effectively and continually educate patients about the need and benefits for active progress through the complete care plan
- Ensure that all of the patient’s needs are met, nothing gets overlooked, and care is consistent with the patient’s expressed desires
- Generate weekly/monthly reports and statements
- Conducting insurance verification, reimbursement inquires, and preauthorization calls
- Additional office tasks including confirmation calls and patient interaction on the phone and in person

EDUCATION & EXPERIENCE
- High school diploma or equivalent.
- 2+ years of work experience in sales (insurance sales, medical device sales, other medical sales, etc.), case management
- Experience working with a CRM preferred
PHYSICAL REQUIREMENTS
- Prolonged periods sitting and working on a computer
- Requires traveling to events in neighboring towns and out of state for training (up to 10% travel)
