Job description
Under the supervision from the Clinical Director of Child and Adolescent Behavioral Health, provides structured, one-to-one, strength-based, and client-centered support services for youth up to the age of 21 who reside at home or in foster care. Supports a specific goal on the youth’s treatment plan, such as developing skills for daily living, social and communication needs. Works collaboratively with other mental health and medical providers engaged in related therapeutic/patient care activities.
Major responsibilities:
- Coaching, supporting, and training youth in age-appropriate behaviors related to social/interpersonal skills, coping skills, problem-solving and daily living skills.
- Engages the youth in activities in the community.
- Offers services in the youth’s home and community.
- Collaborate with a licensed clinician, the youth’s family, and other members of the youth’s clinical team to achieve a specific skill-building goal.
- Create a positive relationship with the youth and family.
- Provide and gather feedback for the youth and family on progress.
- Inform the family of the skills they can implement with the youth at home or in the community.
- Complete necessary documentation.
- Ensures that appropriate referrals and service coordination takes place.
- Participates in any necessary training and supervision.
Minimum Qualifications:
- 21 years of age or older
- Bachelor’s degree in a human service field from an accredited university and one (1) year experience working with children/adolescents/transition age youth; or
- Associates degree in a human services field from an accredited school and one (1) year of experiencing working with the target population; or High school diploma or GED and a minimum of two (2) years of experience working with children/adolescents/transition age youth.
- Valid driver’s license and reliable transportation
