We are a Central Pennsylvania based Non-profit Hospice & Post-Acute Healthcare Organization specializing in home-centered care and we are seeking a talented Social Worker who will be responsible for the provision of medical and social services and supportive counseling in accordance with each patient's plan of care.

Our Social Workers are responsible for the provision of medical and social services and supportive counseling in accordance with each patient's plan of care and will serve our valued clients wherever they may call home; inclusive of Long Term Care and Assisted Living/Personal Care Communities, Hospitals and in the patient's own residence. This position, though office based in Harrisburg PA, is also considered field based; serving our valued clients wherever they may call home; inclusive of Long Term Care and Assisted Living/Personal Care Communities, Hospitals and in the patient's own residence.

All PRN positions have an expectation to adhere to the following guidelines:

  • PRN employees must attend each day of an agreed upon schedule for New Hire orientation.
  • Commit to working a minimum of two full shifts per calendar month.
  • One Holiday per calendar year.
  • PRN employees must attend Annual Hospice In-service days and meet annual training requirements.

Qualifications:

  • MSW from a C.S.W.E accredited program required. At least one year experience in a health-related field. It is preferable that the individual have some experience in relating to issues surrounding death and dying.
  • Support of the Hospice philosophy and mode of care. Willingness to complete HCP Volunteer Training Program.
  • Ability to create and maintain a therapeutic relationship with patients and families while maintaining professional boundaries.

Functions:

  • Perform initial and ongoing psychosocial assessments of patients and families admitted for hospice care.
  • Make referrals to appropriate resources upon request of Team or family.
  • Assist the patient and family with care planning, funeral planning, location of care transfers, financial referrals, anticipatory guidance and safety education.
  • Participate in the development of the overall plan of care for each patient/family in conjunction with the team.
  • Attend interdisciplinary team (IDT) meetings and provide updates relevant to patient/family issues.
  • Document in electronic medical record.
  • Assist the Team in understanding the significant social and emotional factors related to the illness, the family structure, and the family dynamics.
  • Acts as a resource and support to other IDT members concerning pyscho- social issues.
  • Participates in in continuing education and organization training to expand knowledge of end of life and palliative care principles and practices.
