Position Summary

Under the direct supervision of the Director of Behavioral Health, the Senior Behavioral Health Provider (BHP) is responsible for providing trauma-informed behavioral health services at the Zufall Health Center sites. Zufall Health follows the Primary Care Behavioral Health model of integrated care. The BHP provides direct services to patients, families, and groups. They provide brief behavioral health intervention to improve patient health outcomes as well as individual and family counseling and engages in crisis intervention. The BHP contributes to the patient care team through huddles, case consultation, patient advocacy, mental health education, and collaborative team meetings with clinical and support staff. The BHP identifies opportunities for enhancing integrated Behavioral Health Services in the primary care setting and serve as a resource to the rest of the clinical staff.

Essential Functions, Duties and Responsibilities

• The BHP will.
• Have day to day responsibility to ensure that clinical activities are up to the standards of the ZHC policies and procedures and ambulatory care practices
• Along with the Director of Behavioral health, serve as a resource to the site managers regarding BH staffing issues; work collaboratively with other BH staff as well as clinical care team; help with identification and resolution of clinical issues
• Assist the Director of Behavioral Health in clinical performance improvement activities and risk management activities; conduct peer reviews of colleague’s charts as assigned; participate in regular meetings of the BH staff and in collaborative activities, meetings, and initiatives as deemed necessary by the Director of Behavioral Health
• Serve as clinical supervisor for BH providers working towards their clinical licensure as well as master’s level interns and provide support to the BH team when the Director of Behavioral Health is out or unavailable
• Work in collaboration with the primary care team to recognize, manage, and treat patients with mental health disorders, psychosocial problems, and chronic health conditions
• Complete a Comprehensive Assessment and Treatment Plan in collaboration with patients based on patient's history and relative to the presenting problem
• Maintain and ensures that patient records are properly documented and in a timely fashion
• Participate in crisis intervention as response for patients dangerous to self/others/property
• Lead group sessions and activities, and develop curricula for specific initiative (for example, Healthy Lifestyles). Conduct pre- and post-intervention assessments and patient surveys to evaluate effectiveness of programs
• Participate and present in care conferences, patient groups, educational programs for staff and for patients, staff meetings, staff trainings and other venues as assigned
• Represent Zufall Health Center at community service organization meetings, behavioral health agencies and collaborating entities
• Participate in outreach and community educational activities as part of the Care Team as assigned by the Director of Behavioral Health and/or Chief Medical Officer
• Participate in in-service educational programs and continuing education
• Teach and train other health center staff and students as needed
• Follow established departmental policies and procedures regarding quality assurance, safety, environmental and infection control standards. Assist in the development of relevant policies and procedures as the department and services evolve
• Perform other related duties as assigned by the Director of Behavioral Health

Knowledge, Skills and Abilities

• Excellent customer focused interpersonal skills to interact in an effective manner with patients, families, the interdisciplinary health care team and community agencies with diverse opinions, values, and religious and cultural ideals.
• Excellent verbal and written communication skills
• Culturally sensitive and knowledgeable of multicultural mores, beliefs, and customs
• Bicultural and bilingual experience valued and ability to speak both English and Spanish strongly preferred
• Excellent computer skills, experience with electronic medical records a plus
• Ability to work independently or with little supervision and work as a team member.
• Excellent organizational skills such as problem solving, decision making, priority setting, and work delegation.
• Learn and apply the trauma informed care principles with the scope of the position

Education, Training and Experience

• Master’s degree in social work
• Licensed in New Jersey as an LCSW
• Minimum of two years post graduate experience in clinical/ambulatory care setting and three years with LCSW licensure
• Completion of or enrollment in 20-hour certificate in clinical supervision
• Experience in working with groups and with telemedicine preferred
