Position Overview:

This position facilitates admissions services by providing inpatient supportive care and seamless transition to home based care. Assists the organization by assuring that the right care is provided at the right place and the right time. Positively promotes Sutter as the home care provider of choice within the communities for hospice services. Serves as a resource by supporting the mission and vision of the organization.

EDUCATION

Graduate from an accredited school of nursing

CERTIFICATION & LICENSURE

RN-Registered Nurse of California

BLS-Basic Life Support Healthcare Provider

DL-California Drivers Class C

AUTO-Automobile Insurance

PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN:

5 years experience in Hospice setting within last five years experience

5 years experience in nursing experience required.

5 years experience direct patient care

SKILLS AND KNOWLEDGE

Must have clinical knowledge of disease entities common to hospice

Commitment to teamwork and customer service, both internally and externally, is required.

Experience with Microsoft office suite and electronic health record ly desired.

Collaborative and problem solving skills with customers and management.

Organizational skills.

Must have telephone, interpersonal and written communication skills.

Pay Range 63.58 - 84.5

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
