Job Summary:
In accordance with UAHSF established policies and procedures, the Clinical Care Coordinator is responsible for the provision and management of patient care in a clinical area for a specific age patient population. To manage the plan of care for a caseload of patients. To serve as the liaison with the multidisciplinary care team. To plan care management as achieved through coordination of care with physicians, nursing staff, and other health professionals to ensure high quality patient outcomes are accomplished within the established time frame and appropriate use of resources. Coordinates patient follow-up clinic visits, verifies appropriate tests. Under general direction of the physician / APP, coordinates care of patients, serving as a resource for patients, families, physicians, and other health team members in the delivery of care. Obtains information regarding referring physician to assist in continuum of care or return to referring physician for continued care. Provides patient / family education regarding care, medications, and access to additional resources in the continuum of care. Utilizes critical thinking abilities in evaluation of care and the triage of patients. Displays professional rapport with all patients, family and faculty and staff. Displays competencies for basic computer skills, which include MS Word, Outlook, and electronic EHR. Performs other duties in support of specific departmental functions and as deemed necessary.
Position Requirements:
EDUCATION AND EXPERIENCE:
Required: BSN Degree from an accredited school of professional nursing. Three (3) years clinical nursing experience.
Preferred: Five (5) years of nursing experience
LICENSE, CERTIFICATION AND/OR REGISTRATION:
Required: Current AL RN license. Current CPR certification or must be obtained within 30 days of employment.
