Role:

We are seeking a dedicated RN Admissions Coordinator to join our Cedar Ridge Village team! The Admissions Coordinator makes assessment for, plans and advocates for the residents of the community facility so they can reach their highest practical level of physical, mental and psychosocial well-being. The Admissions Coordinator will be responsible for pre-admission assessment of referrals as well as discharge planning for all the residents of the Community. Has the ability to communicate effectively and advocate for resident centered/directed care to team members, families, physicians, allied health professionals and the at large community.

Shift: This position is part of the Nursing Management Team and will regularly work Monday through Friday with rotating on-call and Manager on Duty.

Responsibilities:

• The Nurse Admissions Coordinator follows safety policies in performing nursing care, assuring resident safety
• The Nurse Admissions Coordinator follows infection control guidelines
• The Nurse Admissions Coordinator attends and participates in facility meetings and in-service training programs as assigned
• The Nurse Admissions Coordinator initiates emergency measures according to the community facility policy and within standards of nursing practice
• The Nurse Admissions Coordinator maintains all documentation as required by Federal and State regulations, and the community facility.
• The Nurse Admissions Coordinator assists with meals in the dining room
• The Nurse Admissions Coordinator follows and upholds policies and procedures of the community facility.
• The Nurse Admissions Coordinator follows and understands the community facility's Emergency Preparedness plans
• The Nurse Admissions Coordinator observes residents rights including, but not limited to, privacy, dignity, etc.
• The Nurse Admissions Coordinator promotes positive public relations with residents, family members and guests
• The Nurse Admissions Coordinator coordinates transportation for residents
• The Nurse Admissions Coordinator completes required forms and documents in accordance with company policy and state and/or federal regulations
• The Nurse Admissions Coordinator is responsible for pre-admission assessment of referrals, coordinates admissions and discharge planning for all the residents
• The Nurse Admissions Coordinator coordinates and directs the care plan meetings
• The Nurse Admissions Coordinator facilitates advanced directive decision-making process
• The Nurse Admissions Coordinator coordinates grievance complaint process
• The Nurse Admissions Coordinator coordinates and assist with resourcing and scheduling of allied health services
• The Nurse Admissions Coordinator provides on-going education of residents rights, abuse and neglect prevention to staff

Qualifications:

• Must be a graduate from an accredited school, be currently licensed RN or LPN in state of which applying without encumbrance, and be responsible for maintaining professional licensure and CEU requirements.
• Prefer 2 year experience working in a geriatric setting.
• Must possess a valid drivers license
