I. TITLE: Rehabilitation AideII. JOB SUMMARY: Rehabilitation Aides carry out procedures that improve the functional status of the resident and prevent further deterioration.
III. RESPONSIBLE TO: Director of Rehab
IV. QUALIFICATIONS:
A. Personal:
1. A broad general education.
2. Evidence of good grooming, appropriate manner and conduct.
3. Ability to work effectively with others; demonstrates leadership ability and principles of management.
4. Integrity, imagination, initiative, energy and evidence of self-improvement.
5. Participation in professional organizations and workshops and continuing education.
6. Able to communicate in an effective manner.
7. Flexibility to meet the changing need of the situation.
B. Professional
1. Certified Nursing Assistant by the state of Illinois
2. Must be on the Illinois State Nursing Aide Registry with a clear record
3. Must have a current CPR certification
C. Physical:
1. Evidence of good physical and mental health.
2. Physical appearance: Must present a pleasing image to residents and public and act as an example to employees.
V. PERFORMANCE REQUIRMENTS:
A. Physical Demands: Sits and walks throughout working day. Must have physical stamina.
B. Special Demands:
1. Must be able to project a warm, friendly, sympathetic interest to all.
2. At times work under pressure and tension and hours of duty may be irregular or lengthy.
3. Able to be flexible to meet changing situations.
VIII. WORKING ENVIRONMENT:
Works in a clean well-lighted, temperature controlled office environment.
IX. CONDITIONS OF EMPLOYMENT:
A. A minimum of 40 hours per week.
B. Salary: Based on qualification, experience and education.

X. FUNCTIONS:
A. Support the facility’s philosophy of care and strive to achieve its goals and objectives.
B. Responsible for the maintenance of rehab supplies and equipment.
C. Cares for and applies or supervises the application of orthoses, braces, prostheses, and any other appliance or assistive device.
D. Provides hand on support to implement the specific individually designed rehab programs.
E. Documents daily participation and completes monthly progress notes as needed.
F. Monitors walk and dine program and other rehab programs.
G. Demonstrates knowledge of resident safety and abuse prevention programs.
H. Recognizes and reports any changes in resident’s condition to the charge nurse.
I. Participates as a team player.
J. Answers call light promptly.
K. Attends and participates in care plan meetings.
L. Assists in serving and feeding residents.
M. Assists families and visitors when appropriate.
N. Follow established fire, disaster, and evacuation policies and procedures.
O. Perform the duties of CNA’s as needed for patient care.
P. Perform all other duties as directed by the Director of Rehab.
