The Home Health RN Case Manager for Gallagher Home Health Services are afforded flexibility, independence, and autonomy. Our Home Health nurses make great wages and are able to increase their earnings with improved patient outcomes, lower readmissions, and additional visits.

Responsibilities of the RN Case Manager Home Health:

• Coordinate and provide Primary Care for our patients to ensure safe, timely, effective, and efficient care.
• Manage your own patient schedules.
• Case Conference weekly with your direct Manager and determine ongoing care or Discharge and Recert Planning.
• Direct Patient Care to help them achieve wellness and autonomy
• Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
• Promote quality and cost-effective interventions and outcomes
• Adhere to professional standards as outlined by protocols, rules, and regulations
• Perform Timely and Accurate electronic charting that reflects the care provided each visit

What we are looking for:

• Covid-19 Vaccination or willingness to obtain Covid-19 vaccination
• One (1) year of RN experience
• Current license, certification, or registration
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
• Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplied and equipment. Considerable reaching, stooping, bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills
