JOB TITLE: Registered Nurse
WORK HOURS: 8 a.m. – 5 p.m. Monday – Friday as well as rotating On-Call
STATUS: Non Exempt
DEPARTMENT NAME: Clinical
PHYSICAL LOCATION: The main office is located in Lynnwood, WA. Clients are seen in their homes in Snohomish, King, and Pierce counties.
REPORTS TO: Clinical Director and Administrator.
The Registered Nurse Case Manager (RN) is responsible for the day-to-day skilled nursing care of clients receiving services from EverHome Healthcare (EHHC). A critical component of the nurse’s role is to demonstrate the company’s culture of compassion, accountability, and results by displaying the highest standards of clinical integrity, superb follow-through and consistency, and exceptional customer service and client satisfaction. The RN works as a member of the clinical team comprised of care specialists, the medical director and rehabilitation professionals, and the case manager. The team assures that all of the client's needs are being addressed in a timely and clinically expert manner.

- Must be a licensed Registered Nurse in the State of Washington.
- Must be a graduate of an accredited School of Nursing.
- 3 years of clinical experience; preferably in rehabilitation, home care, or hospice.
- Must demonstrate an ability to practice independently in the delivery of nursing care.
- Must have completed Nursing delegation training
- Ability to read and write in English.
- Require a reliable vehicle for use when traveling to work sites.
•A valid Washington State Driver's License
- Proof of current auto liability insurance
- Computer literate with experience in different computer programs
- Strong grasp of legal and ethical issues pertaining to confidentiality.

DUTIES, RESPONSIBILITIES, and ACCOUNTABILITIES: The following duties and responsibilities reflect the essential functions for this job, but do not restrict the tasks that may be assigned. The Director may assign or reassign duties and responsibilities to this job at any time due to reasonable accommodation or other reasons.
•Performs the essential activities of Skilled Nursing including assessment, planning, implementation, coordination, monitoring, evaluation, outcomes, and general interventions of skilled nursing practice and process, and makes appropriate referrals to other disciplines.
- Performs initial evaluation and assessment of client needs
- Works with the Team to establish the Care Plan with team and coordinates and implements appropriate alternatives
- Observes client for changes in condition and reports such changes to the Physician and/or Clinical Supervisor, obtains orders for changes to the Care Plan, and documents all observations and interventions in the clinical record.
- Develops and keeps current orders for medications and treatments as prescribed by the Care Plan
- Implements and revises the client's Plan of Care with Physician approval as appropriate.
- Uses appropriate preventive, therapeutic, and rehabilitative nursing procedures and keeps the client's physician, family, and Clinical Supervisor informed of the care being provided.
- Assists and instructs client and family members in nursing care required to promote the client's health and safety.
- Documents nursing observations, interventions, and evaluations of the care being provided in accordance with agency policy.
- Participates in team conferences, as requested, and attends staff in-services programs.
- Provides Nursing Delegation to appropriate staff and supervise same to assure compliance with the delegation and safety of the clients.
- Thoroughly explains the Company’s mission and value to the client and the client’s family members, doctors, providers, etc.
- Explains the nurse’s (RN) role and services of the Company, describes client’s rights, and provides contact information for complaints
- Research client information regarding diagnosis and treatment options as requested
- Researches medical history, care and/or equipment costs, community and/or government programs, and other information that would contribute to or enhance cost-effective quality care for the patient.
- Responsible for accurate, timely, and ongoing evaluation and reporting of client’s needs, services, and outcomes
- Monitors and documents weekly progress of client issues, goals, and outcomes, documents findings and appropriate cost-effective recommendations
- Documents assessments, progress, re-evaluations, billing, and other pertinent information in the client’s documentation system
- Performs other duties as required.
