POSITION SUMMARY: The Quality Improvement Coordinator assists the Supervisor of Quality Improvement with the implementation and coordination of quality improvement plans and programs. This position utilizes standards of practice to teach clinical staff the safe delivery of high-quality care to adult and geriatric clients in accordance with agency policies and procedures and State and Federal Guidelines.
POSITION REQUIREMENTS: Must be a graduate of an accredited RN program. Current RN license and maintenance of license in the State of Connecticut. Minimum of 2 years of full-time homecare/hospice experience required 3-5 years preferred. Previous educational or teaching experience preferred. Excellent communication, interpersonal, computer and customer service skills. Demonstrated OASIS knowledge and expertise. Must be willing to travel to all three office locations as necessary.
QUALITY IMPROVEMENT RESPONSIBILITIES:
- Performs concurrent and retrospective clinical record reviews to assess the appropriateness of the plan of care, proper utilization of services, and reviews documentation for accuracy, completeness, and consistency.
- Works collaboratively with members of the interdisciplinary team by providing ongoing education and feedback to assure documentation is complete and consistent with care and reflects legal requirements.
- Participates in focused, routine, and quarterly clinical record reviews, for both homecare and hospice, as required utilizing an applicable clinical record audit tool. Identifies aberrant practice trends in documentation, reports findings, and prepares recommendations for operations.
- Provides ongoing training to field staff regarding documentation of assessments, care planning, implementation, and evaluation of outcome measure achievement.
- Assists with staff development, in-services, and laptop training to maintain compliance with organizational and regulatory requirements. Functions as a clinical resource to personnel and may conduct joint visits with clinicians in the field if necessary.
- Develops, coordinates, and conducts an appropriate effective orientation training program for new employees, and ongoing training for all employees as required, for both hospice and palliative programs.
- Participates in interdisciplinary team meetings, reviews documentation, and assesses appropriateness for palliative level of care, and works with appropriate leadership to provide ongoing feedback, follow-up, and education to applicable team members.
- Assists with tracking and instructing staff on infection, incident, and complaint processes, reporting, and other related mandatory trainings.
- Participates in and assists with community activities as assigned.
- Maintains knowledge of current Medicare and Medicaid rules and regulations, as well as quality and educational compliance issues as regulated by the State of Connecticut.
- Performs all other related duties and requirements as deemed necessary and appropriate.
