*Required Qualifications*
- Registered Nurse with a NY License, and no disciplinary action
- 2 years of experience of in home car
- Experience conducting orientation for new hire HHAs
- Knowledge of Home Care Regulation
- Experience working with the adult population
- Knowledge of community health and social service agencies and additional community resources
- Exceptional communication and interpersonal skills with the ability to quickly build rapport
- Ability to work with minimal supervision within the role and scope
- Ability to use a variety of electronic information applications/software programs including electronic medical records
- Effective computer skills and experience with Microsoft Word, Outlook, and Excel
- We will require full Covid vaccination for this position.
*Preferred Qualifications*
- NYS RN license, cpr, physical, drug screen and ppd/chest xray
- 2-4 years of in home assessment and care coordination experience
- Bilingual a plus
Pay Rate $45-$47/hour
