The Program Coordinator at HSI will provide client services to our residential shelter Men’s and Safe Haven program participants. This role provides opportunity for an outstanding direct care experience working side by side with our talented staff who are passionate about fulfilling our mission and helping our clients meet their service plan goals and achieve self-sufficiency.
This role is a full time exempt onsite position, with opportunity for flexible scheduling based on experience and at management discretion. This role may be offered with the opportunity for alternative work scheduling of either a 4 day work week (onsite) or a 5 day work week with one day of remote work, in accordance with current organizational policy. Applicable candidates may be eligible for employer funded LCSW supervision as part of their employment and subject to availability.
1. Collaborating with the Division’s Admissions Coordinator in order to assess benefits history and barriers for each new guest upon admission into the shelter.
2. Meeting individually with guests in the Single Men and Safe Haven programs in order to assess needs related to income and benefits and provide appropriate referrals and support, which may include, but is not limited to:

  • Ensuring that new guests are linked with the Office of Temporary Assistance (OTA) within their first 48 hours in the shelter in order to be screened for and linked to appropriate public benefits (including Emergency Assistance/Immediate Need benefits as it relates to bed night reimbursement).
  • Assist consumers in overcoming any barriers that will hinder their progress in the program. This may include transportation assistance, accessing professional clothing and/or uniforms, or obtaining required documentation such as photo ID’s, social security cards, etc.
  • This may include providing transportation to guests as needed with agency vans and working collaboratively with the RSS Division Manager and other staff in order to link guest to regular transportation assistance.
  • Providing support and navigation assistance for guests who are unable to work due to disability, etc. and have applied or are applying for Social Security benefits.
  • Providing necessary support, advocacy, and navigation assistance to guests who are eligible for, entitled to, or receiving other forms of income in order to ensure access to and maintenance of applicable entitlements.
  • Assisting guests with applicable recertification processes for income sources and public benefits, including Medicaid/Medicare.
  • Troubleshooting issues related to guest benefits and income including medical insurance in order to ensure adequate, uninterrupted coverage.

3. Establishing and maintaining strong working relationships with external agencies responsible for administering benefits and providing support to guests as it relates to income and benefits including, but not limited to: OTA, the Department of Labor, Employment & Training Services, the Department of Human Services / Medicaid, the Social Security Administration, the Department of Children and Families, and other applicable entities. This includes creating, managing and continuously enhancing a network of employers who are willing to provide employment opportunities, training and career support to HSI guests.
4. Maintain up to date resource guides and other information on community resources and partner agencies to share with other staff and guests as needed.
5. Working collaboratively with other team members within the Men and Safe Haven Division and other HSI staff in order to ensure seamless provision of services and complete wrap around support for every guest within the division. This may include following up and addressing incident as they occur as well as providing as needed crisis intervention and supportive counseling.
6. Work closely with other staff to arrange for transportation and other support for guests so they can attend medical, social service and housing related appointments and job interviews/fairs.
7. Communicate regularly and clearly to Residential Support Staff and other team members in order to ensure seamless provision of services.
8. Serve as agency liaison for all volunteer career coaches, including scheduling career coaching sessions monthly for guests/participants in all HSI programs and communicating with volunteers prior to and after sessions to provide applicable information and follow up support.
9. Work collaboratively with Volunteer Coordinator to ensure successful and positive experiences for all volunteers providing career coaching assistance to guests. This may include providing guidance and direction for volunteers assisting with various onsite tasks.
10. Collaborate with the housing specialist and other team members in order to help guests set realistic housing goals based on their income / income potential and other barriers.
11. Regularly collecting proof of income and benefits from all guests and appropriately maintaining such documentation in guest files and HMIS where it can be easily accessed by all staff within the Division as needed.
12. Attend monthly floor meeting for division and help other team members plan for and follow up on meeting topics.
13. Attend county Provider Resource Committee meetings and other applicable community events and committee meetings as requested by Management in order to stay up to date on available resources for guests and maintain strong relationships with other providers.
14. Participating in required cross training in order to provide as needed support to other roles and departments. This includes flexibility in working with management to adapt to organizational changes.
15. Document all client interactions and follow up work in HMIS in a timely and organized manner.
16. Assist guests in other division programs as requested.
17. Complete special projects at management direction.
18. Establish and maintain constructive working relationships with guests and colleagues.
19. Participate in all staff meetings and related trainings.
20. Exhibit strong personal accountability for, awareness of and adherence to company standards, policies, procedures and processes in all aspects of employment with HSI.

Requirements

A. Bachelor’s Degree in a related field preferred
B. Minimum one year experience working with homeless population
C. Valid driver’s license and clean driving record
D. Satisfactory results of periodic background checks
E. Basic knowledge of applicable software and technology including Microsoft Office Suite products
F. Strong interpersonal and communication skills. Ability to communicate clearly both verbally and in writing
G. Familiarity and knowledge of fair housing laws
H. Accuracy and organizational skills
I. Regular and predictable onsite attendance during the scheduled hours for this role is an essential function
J. Ability to attend occasional offsite and off hour meetings as required by the duties of this role.

1. Frequent standing and walking including stairs.
2. Ability to stand for significant periods of time without rest.
3. Regular lifting of up to 30 pounds.

Additional Information
Full time permanent staff of Homeless Solutions are eligible to enroll in fully paid medical, dental and life insurance coverage plans in accordance with HSI policies.
