We are seeking a PHYSICAL THERAPIST in Medford, WI at Aspirus Medford Home Health who provides physical therapy to adolescent, adult and geriatric patients. Evaluates, establishes treatment plan and goals of treatment, and provides for and/or supervises the administration of treatment modalities/procedures done to meet the needs of the patient as it pertains to the age and diagnosis of the patient. Incorporates patient and family education. Provides documentation for all of the above.

PROFESSIONAL EXPERIENCE/QUALIFICATIONS

• Bachelor’s degree in physical therapy from a school of Physical Therapy accredited by the American Physical Therapy Association (APTA).
• Clinical Specialist certification beneficial.
• Possesses automobile liability insurance with minimum $100,000/$300,000 coverage required, or $300,000 combined single-limit liability.

LICENSURE

• Current Wisconsin and/or Michigan licensure to practice Physical Therapy required.
• Current BLS certification or obtained within 90 days of hire.
• Possesses and maintains a valid Driver’s License required.

HOURS

• 80 hours every 2 weeks
• Monday-Friday day shift
• Weekend shift is every 5th-7th

Aspirus at Home is a part of the Aspirus health system and is a nonprofit, Medicare-certified home health agency that serves 14 north central Wisconsin counties. We offer services in north central Wisconsin and parts of the UP. Aspirus at Home helps to bridge the gap between the more intensive levels of care received in a hospital, nursing or rehabilitation facility and in client's home.
