Seeking a Nurse Practitioner, Diabetes Services, as a professional practitioner, assumes responsibility for the coordination of the outpatient clinic Diabetes Education Program. Conduct diabetes education for patients, staff, and families including diabetes medications, oral and injectables, insulin pumps, continuous glucose monitors, and other educational needs. Develop individualized education plans and resource materials that measure patient progress and take place in the outpatient setting.
The Family Nurse Practitioner will also be prepared to provide services to non-diabetic clinic patients following adopted clinical protocols under the supervision of licensed physicians. Will follow current treatment protocols for the services to be provided with assistance and consultation from the clinic physicians and will assume personal responsibility and accountability for decisions regarding initiation of, or changes in, treatment of their patients with guidance from their supervising physician.

Essential Function
- Sweet Success Program – Facilitate
- Complete California Diabetes and Pregnancy Program (CDAPP) Sweet Success required provider training
- Develop and disseminate CDAPP Sweet Success Guidelines for care for women with diabetes before, during, and following pregnancy
- Ensure the outpatient clinic Sweet Success program meets CDAPP requirements including patient guidelines, goals and objectives, provider and training, and annual site visits or surveys
- Diabetes Specific Accountability
- Collaborate with Comprehensive Perinatal Services Program (CPSP) staff and providers on co-managed patient population
- Serves as a resource, role model, and consultant for all clinics and staff regarding diabetes management issues
- Responsible for collecting, compiling, and interpreting data on outpatient diabetes management audits and preparing reports for committees
- Participates in community outreach related to diabetes prevention and self- management
- Participates in the development and implementation of performance improvement activities, both clinically orientated and related to the administrative process
- Serves as an advocate and liaison for patients, families, and staff regarding diabetes management including diabetes survival skills and self-management
- Maintains competency skills including the ability to provide care to special patient populations and patients with diverse cultural backgrounds
- Conducts and participates in research focused on diabetes management across varied patient populations
- Ability to effectively help participants change and maintain dietary and other lifestyle behaviors
- Clinical Services
- Performs procedures as qualified by training, specialty, and protocol with supervising physician oversight and approval.
- Takes complete histories and performs physical examinations, or those assessments pertinent to the reason for the visit, in order to provide a patient data base and/or basis for a therapeutic or health maintenance plan.
- Orders appropriate laboratory tests, x-rays, EKG, and comparable procedures, under established protocols.
- Diagnoses and treats common illnesses and injuries under established protocols.
- Practices in accordance with Medical Staff bylaws and guidelines.
- Records health, social, and other pertinent problems and data in the medical records.
- Orders routine medications and treatments under established protocols for acutely and chronically ill patients under the supervision of the physician and established protocols.
- Works with other staff in the establishment, evaluation, and monitoring of treatment programs.
- Follows all policies and procedures (medical and fiscal) when completing medical charts, billing forms, and other required paperwork.
- Reviews protocols and patient care activities with supervising physician annually.
- Reviews all referrals with supervising physician where appropriate.
- Participates in quality assurance and utilization review.
- Quality Focus
- Maintains high quality by self-monitoring patient care activities, staying up to date in the area of specialty/practice, and participating in MMHS quality improvement programs.
- Acts as a resource person for “triage” questions, patient case management, and assessment.
- Provides staff and patient education as needed.
- Enhances professional practice by participating in problem-solving with management, nursing, and clerical staff.
- Serves as a member of selected committees as assigned.
- Notifies management of instrument and/or procedural problems within specified timeframes.
- Works cooperatively with management and staff to ensure optimum patient flow and to provide that patients are attended to in an efficient and timely manner.
Requirements
Education:
- Completion of the Accredited Nurse Practitioner program
- Skill Requirements:
- Ability to communicate effectively
Licensing and Certification Requirements:
Diabetes Specialty –
- Certified Diabetes Care and Education Specialist (CDCES), credential must be completed within 2 years from hire or at the completion of providing 1,000 hours of patient diabetes education
- Experience in nutritional and diabetes counseling and education
- Strong skills in assessing, designing, implementing, and evaluating the nutritional, lifestyle, and educational needs of patients and program participants
General NP requires -
- Current License as Registered Nurse/Nurse Practitioner with the Board of Registered Nursing in the State of California.
- Successful completion of an accredited nurse practitioner program of study that conforms with the Board’s educational standards set forth in the California Code of Regulations Section 1484 for California graduates only.
Or
- Current certification by one of the following boards: the American Nurses Credentialing Center or American Academy of Nurse Practitioners or Pediatric Nursing Certification Board or National Certification Corporation for the Obstetric, Gynecologic, and Neonatal Nursing Specialties, or the American Association of Critical-Care Nurses.
- Current AHA BLS certified
- Current AHA ACLS certified (if submitting privileges for inpatient setting)
