ABOUT US
Transition Projects connects individuals with the services, resources, and tools they need to end their homelessness. Our team of over 350 assists more than 10,000 people each year at 14 different sites in the Portland metro.
- On any given day, we help meet the basic needs of more than 500 people experiencing homelessness through our Resource Center.
- On any given night, we provide a safe place to sleep for more than 800 people with nowhere else to turn.
- In any given year, we help place more than 1,000 people into affordable housing – and then support them in retaining that housing.

Starting Salary Range: $23.50/hr
GENERAL RESPONSIBILITIES
This position provides street outreach to people experiencing homelessness in the central city area. This position coordinates with the Navigation Team to assist people to access shelter, housing, and connect to needed support services.

SPECIFIC RESPONSIBILITIES
- Provide engagement and outreach to people camping in the central city area.
- Provide education and resources to encourage use of existing services for hygiene, shelter, and basic needs.
- Coordinate with community social service partners for linkage to medical, mental health, and substance use disorder services, and culturally or youth specific services when needed.
- Escort and advocate for participants referred to medical, mental health, and substance use disorder programs who might otherwise not make the connection to services.
- Provide follow-up services on-site at the Resource Center for engagement of participants who present for services.
- Other duties as assigned.
MINIMUM QUALIFICATIONS
The right individual for this position has a passion for helping others and a desire to work in an organization that serves people who are experiencing poverty and homelessness.
- Applicant must have completed Transition Projects Mentor Program, a certified Peer Support Specialist program, or equivalent with documentation.
- Applicants who are themselves veterans will receive priority consideration. Bi-lingual English/Spanish strongly preferred.
- Demonstrated ability to engage people experiencing homelessness in a trauma informed manner.
- Demonstrated ability to maintain paperwork in an organized manner.
- Demonstrated ability to use word processing, database, calendar, spreadsheet, e-mail, and Internet.
- Ability to operate office machines, including copy and facsimile machines.
