Summary

THIS JOB WILL BE COMPLETELY TELEPHONIC. NOT DIRECTLY FACING WITH PATIENTS.

The Patient Care Coordinator supports patients, families, and physicians to promote timely access to needed care and ensure continuity of care. S/he will provide a broad range of services to patients and their families under the direction and supervision of the Quality Manager. The Patient Care Coordinator will assist patients in identifying and obtaining available services, resources, and benefits. The Patient Care Coordinator is responsible for supporting care management efforts for all patients with recent hospital episodes. The Clinical Quality Data Coordinator will serve as a facilitator and catalyst for improving the clinical and operational performance using the Patient Centered Medical Home (PCMH) model of care within the practice. Clinical duties may include recording/updating medical record documentation and medical histories. Preparing patients for appointments and follow-up.

Job Duties

Contributes to care management and care redesign of population health efforts

Process daily Hospital Discharges from targeted hospitals

Perform patient outreach daily to schedule hospital follow-up appointments

Perform alternative contact methods for patients unable to be reached by phone

Review patient hospital discharge documents and track communication outcomes in excel

Assist RN Care Managers in monitoring patient care plans

Provide community resources to patients that have social determinant needs

Improve patient experience including patient safety and quality of care provided

Yearly Care Management Continuous Education courses required

Qualifications and Education

High School Graduate or GED

Medical Assistant Certificate preferred

Must complete Care Management Courses with Michigan Care Management Resource Center within 2 months of hire

Minimum two years’ experience in a healthcare setting

Ability to read and comprehend patient medical charts and medical terminology

Job Type: Full-time

Pay: $14.00 - $16.00 per hour
