SUMMARY OF JOB ROLE:
The Marketing Liaison (ML) is responsible for building and maintaining customer relationships for referral partners in their assigned territory/specialty. The Marketing Liaison is responsible for providing customer service to ensure performance meets expectations and to increase referral volume and expand market share in the communities GHHS serves. The ML will participate in community education activities, engage in community outreach efforts and build business commensurate with the agency’s strategic plan. The ML will facilitate communication between admissions, the care team and external customers, ensuring that operations are smooth, relationships are solid and service recovery issues are cared for promptly and appropriately.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
- Markets the services and programs of home health care, palliative care, hospice and home care to key referral sources including physicians, nursing homes, assisted living facilities and hospital personnel including but not limited to case managers, discharge planners and other appropriate referral sources.Develops and manages accounts, facilitates dialogue between the GHHS staff and referral partners to ensure seamless care delivery and communication from initial referral through each patient’s discharge.
Meets with patients prior to discharge to coordinate care and define resources to ensure safe transition to home.
Obtains pertinent demographic and medical information for writing referrals to ensure appropriate care can be initiated upon patient discharge. Assists with delivering orders for signature to physician offices.
Develops and conducts educational programs and related printed material. Participates in community education and outreach efforts.
Orients, supervises and directs the Transitional Care Liaison assigned to their territories.
Works closely with the Intake coordinators to farm out cases the agency is unable to accept related to staffing issues, geographical location or insurance coverage.
Monitors and reports any feedback from referral sources, communities, patients or customers that may positively or negatively affect agency operations.
Actively participates in quality assurance and performance improvement (QAPI) teams and activities.
Works independently but willingly assists other team members as necessary to accomplish company goals, customer service and client needs as required.

Maintains accurate expense reports and accounting for market activities.Willingly accepts other duties as assigned.
SUPERVISORY RESPONSIBILITIES
This position assists the Regional Director of Marketing with the supervision of assigned Transitional Care Liaisons.
Knowledge of:
- GHHS services and philosophy
- Insurance Reimbursement including Medicare and Medicaid
- Assessment protocol and admission criteria for GHHS services
- Microsoft Office software packages, Internet, Email
Ability to:
- Demonstrate excellent clinical judgement, critical thinking, problem solving and planning skills
- Work efficiently and accurately
- Follow County, State, and Federal Regulations
- Communicate effectively verbally and in writing
- Establish and maintain effective working relationships with coworkers, referral sources, patients and their families, and physicians

Physical Demands
While performing the duties of this job, the employee is regularly required to stand, walk, sit; use hands to finger, handle and/or feel; reach with hands and arms; and talk or hear. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, computer work and ability to adjust focus.

Pay: $40,000.00 - $50,000.00 per year
