Job description

  • Ensure the provision of customer focused care and safety in the med-surg setting. Carry out responsibilities within the framework of the established standards of patient care in nursing practice.
  • Participate as a vital team member of the inpatient Rehab care team. Monitors the interdisciplinary components of the inpatient rehab programs to ensure they are consistent with the overall strategic plan of the hospital. Will be the primary resource for patients and their health care partners in navigating the health care experience for rehab health issues. Assist the rehab and support staff with communications and education of patients and their families. Resource professional to hospital and office staff throughout the Riverview Health campus. Case management for all physicians as necessary. Maintain confidentiality of a variety of subject matters.

JOB DUTIES AND RESPONSIBILITIES

  • Collaborating with multi-disciplinary departments and providing expertise and guidance as needed for rehab patients
  • Incorporate "best practices" on the Rehab Unit
  • Maintains relevant policies and procedures, patient education, outcomes data collection, quality improvement efforts and education of relevant staff with focus on patient safety.
  • Collaborate with Hospitalists, Primary Care Physicians and marketing department for educational sessions, community events and patient symposiums.
  • Provides education to the patients and families regarding their hospital stay and discharge instructions.
  • Acts as a liaison between patient, physicians' offices and nursing units for delivery of care.
  • Provide education as needed to nursing staff.
  • Responsible for contacting patient and following them through their plan of care for compliance, questions, assistance with medical terminology, etc.
  • Ability to interact directly with physicians, patients, referring physicians and hospital departments.
  • Responsible for working in conjunction with Director and Manager to plan patient retention strategies.
  • Must possess strong time management skills to meet time sensitive deadlines.
  • Responsible for triaging physician concerns brought to employee either formally or informally to department Director in a professional and timely manner for follow up.
  • Responsible for complying with all applicable hospital and departmental policies and procedures, including, but not limited to all departmental and functional policies and procedures.
  • Responsible for supporting the goals and objectives of the department.
  • Attends pertinent and assigned committee meetings.

  • Administer prescribed interventions, both medical and nursing when qualified.
  • Answer patient call lights
  • Make frequent rounds of all assigned patients to determine condition and needs

JOB REQUIREMENTS

EDUCATION AND EXPERIENCE
  • Level of Education
    • Minimum: Graduate of accredited School of Nursing
  • Level of Experience
    • Minimum: New graduate
    • Preferred: Hospital experience

  • State Issued License - Yes
  • CPR - Yes
  • ACLS - N/A
  • PALS - N/A
  • Type of Specialty Certification or other Licensure, if required: - Indiana LPN
