The Mary T Family of Companies has an opening for a full-time Licensed Practical Nurse (LPN) in our ICF licensed group home located in Coon Rapids, MN.

Licensed Practical Nurses are responsible for conducting a focused assessment of the health status of an individual, and reporting changes and responses to interventions. Responsibilities also include implementing interventions and assigning and monitoring nursing tasks or activities to unlicensed assistive personnel. Responsibilities include:

• This position is responsible for conducting a focused assessment of the health status of an individual; through collection and comparison of data to normal findings and the individual's current health status. Required reporting of any changes and/or responses to interventions, in an ongoing manner, to a registered nurse or the appropriate licensed healthcare provider for delegated or assigned tasks or activities.

• Participation with other health care providers, in the development and modification of a care plan
• Determining and implementing appropriate interventions, within a nursing plan of care, when delegated, ordered, or assigned by a registered nurse/licensed health care provider
• Assigning and monitoring nursing tasks/activities to unlicensed supportive personnel; promoting a safe and therapeutic environment; advocating for the best interests of individual
• Assisting in the evaluation of responses to interventions
• Collaborating and communicating with other health care providers
• Providing health care information to individual’s, staff or other interdisciplinary team members
• Providing input into the development of policies and procedures
• Accountability for the quality of care delivered, recognizing the limits of knowledge and experience; addressing situations beyond the nurse's competency; and performing to the level of education, knowledge, and skill ordinarily expected of an individual who has completed an approved practical nursing education program
• Responsible for the compilation and presentation of health care reports

• Experience in an ICF licensed group home is preferred
• Must possess a valid driver's license with a good driving record
• The ability to pass a background check
• An active LPN license in Minnesota
• Strong verbal and written communication skills
• Basic computer skills
