$33 – $43 Per Hour (DOE)

+ Sign-on Bonus (Full-Time employees)

+ $3/hr Evening Differential

+ $5/hr NOC Differential

+ $2/hr more for all shifts Saturday and Sunday

JOB SUMMARY:
A Licensed Practical Nurse (LPN) working in our Skilled Nursing facility is responsible for the daily nursing care needs of our residents. This position is a valuable clinical team member contributing to the smooth, organized and efficient operations of the nursing department.

RESPONSIBILITIES:

• Provide primary nursing care to residents with varying backgrounds and medical histories
• Assists with new admissions
• A Licensed Practical Nurse (LPN) will monitor and communicate changes of condition to provider
• Provide medical intervention as needed for emergent needs
• Communicate needs of resident to outside providers including but not limited to: Primary Care Provider, Home Health Agency, Pharmacy, etc.
• As a Licensed Practical Nurse (LPN) you would communicate resident condition to family, and or responsible party members
• Implement and follow Primary Care Providers orders
• Other Duties as Assigned

REQUIREMENTS:
Licensed Practical Nurse (LPN) Licensure in the State or practice
