JOB PURPOSE:
The In-Home Outreach Counselor - Family Preservation program provides in-home/outreach counseling services and case management for clients/families in the DCFS Family Preservation Program in the San Gabriel Valley.
ESSENTIAL JOB FUNCTIONS:
- Provide child risk assessment for possible child abuse and neglect for each child in the Family Preservation Program at the child's home.
- Consult the DCFS Child Abuse Hotline as needed and complete report within 24 hours.
- Collaborate with DCFS CSWs and Dept. of Probation/Probation Officer in a team-based approach.
- Provide weekly in-home counseling services and/or child follow-up visits as well as provide any supplemental services as needed when needed.
- Provide crisis intervention (3 hour response, 24 hours a day, 7 days a week to resolve emergent issues/situations which threaten the stability of the home and/or safety of the child[ren] to remain in the home) via cell phone and subsequent face-to-face response, as needed and appropriate.
- On call 24/7 and carry agency cell phone to respond to client, CSWs, and agency phone calls.
- Provide case management services and link clients to all indicated services.
- Meet service rate and documentation/paperwork quantity/quality expectations and deadlines.
- Continuous consultation with manager and relevant supervisors to meet clinical quality expectations.
- Initiate, organize, and complete all MCPCs for assigned clients.
- Complete all Auxiliary and Discretionary Fund Requests.
- All other relevant duties as assigned by Program Director and Clinical Supervisor - Family Preservation.

WORK SCHEDULE: (subject to change based on Center’s needs)
Work-Week: Sunday – Saturday
- Actual work schedule as agreed with Direct Supervisor and based on business/program needs.
- Looking for both full time and part time positions. Full time would be benefited position.
MINIMUM QUALIFICATIONS:
- Master's Degree in Social Work, Psychology or Related discipline required;
- Minimum of two years of practical clinical experience or a Bachelor Degree and enrolled in a Master's Program with at least 4 years of field and clinical experience and related educational background
- Demonstrated ability to write and speak English/Spanish well, English/Spanish proficiency preferred.
- Must maintain a valid California driver’s license, insurance and driving record which complies with agency’s insurance carrier standards.
- Requires the ability to independently use a computer, possess good communication/computer skills, have Microsoft Office skills, and learn necessary computer programs.

Pay: $21.50 - $25.00 per hour
