The Home Health RN Lead is responsible for training all nursing staff, covers for leaves/PTO, and works on agency special projects. The Home Health RN Lead is responsible for ensuring that care and services are delivered appropriately.

• Reviews referrals and available patient information to determine patient needs and assign appropriate staff.
• Reviews and evaluate patient care as indicated through record review, case conferences and consultation with staff and instructs and guides clinicians to promote more effective performance and deliver of quality home care services.
• Will be available during operating hours to assist clinicians as appropriate
• Reviews patient’s clinical diagnosis, medications, procedures and clinical course.
• Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing the plan of care.
• Attends case conference meetings with organization personnel to facilitate coordination of care.
• Conducts quarterly record reviews and communicates findings and recommendations to supervisor and appropriate organization personnel.
• Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals. Assists in the orientation and training of new organization personnel.
• Assists DPS/Clinical Manager in the planning, implementation, and evaluation of in-service and continuing education programs.
• Assists in the implementation of P&Ps as well as strategic goals and objective.”
• Complies with accepted professional standards and principles.
• Participates in public relation and community activities that promote the Organization's role as an effective member of the health care delivery system.
• Promotes customer service orientation to all organization personnel.
• When needed is responsible for covering patient visits when organization personnel are out of leave and or PTO.
• Assists with clinical bedside education and training of organization personnel and/or defined by QAPI goals and expectations.
• Performs other duties, activities and special projects as delegated by the DPS/Clinical Manager.

• Requires a current RN license in the state where employed
• Prefer two years’ experience within the last five years in a Home Health agency primary care clinic, or health facility, or MD.
• Must possess working knowledge of state and federal home health regulations.
• Excellent verbal, written and computer literacy skills in Outlook, Word and Excel at a minimum.
• Direct patient care as needed.
• Ability to maintain assigned work hours: Has sufficient endurance to perform tasks over long periods of work hours.
