Healthcare Coordinator (Registered Nurse)

The Healthcare Coordinator position serves as key liaison, advocate, and resource of information regarding routine and special healthcare for each person supported by the agency. This position provides clinical consultation regarding people with intellectual and developmental disabilities supported by the agency, with specific attention to individual health risk factors whose mitigation impacts diet, medication and/or medical treatment/medical safeguarding devices. This position also provides assistance to the Continuous Quality Improvement process including management of the Medication Committee.
Requires clinical competencies and experience in the health care field (Registered Nurse; experience with special needs population).

· Provides clinical services to ensure individuals in care are receiving relevant, competent, and compassionate care that meets their needs.

· Provides valuable input into the agency’s continuous quality improvement efforts to ensure it maintains full adherence to regulatory policies and procedures.

· Maintains timely, detailed documentation to ensure adequate funding levels are met.

· Records individual progress which will ensure continuity of support to individuals served.

· Must be willing and able to work a flexible schedule as assigned or established to ensure the needs of the individuals are met and monitored, including on-call status.

Job Type: Full-time

Pay: $70,000.00 per year
