JOB ANNOUNCEMENT

JOB TITLE: Family Navigator

REPORTS TO: Program Manager

JOB LOCATION: Solano County, primarily FBP Vallejo Main office

WORK SCHEDULE: WORK SCHEDULE: 40 hours per week (Includes weekends and evenings)

STATUS STATUS: 1.0 Non-Exempt

SALARY: Range $24-$30 per hour

Fighting Back Partnership’s Mission:

Vallejo Fighting Back Partnership (FBP) is a non-profit organization committed to preventing and ending poverty and its effect in Vallejo and throughout Solano County.

Family Navigator:

The Family Navigator is responsible for providing wraparound case management navigation and referral services to parents and their families, including advocacy for services within the community, health, and social services system, along with resources for advancing educational and workforce opportunities. This is an essential position that creates linkages for transitioning clients from basic services to viable programs designed for advancing sustainable and quality lives; particularly for Solano County families with children ages zero to eighteen.

ESSENTIAL FUNCTIONS:

• Safeguards the dignity and protects the rights and individuality of each parent and family member served.
• Provides all levels of needed assistance to the parent and family, including assistance in gaining access to public and private community resources, determining eligibility, obtaining services and interpreting policies and procedures for programs serving families.
• Involve the parent and family members in case and/or treatment plan development and promote understanding.
• Develop and monitor working case plan and/or treatment goals and objectives with measurable outcomes.
• Provide assessments and formulate case plans for parents, children and families.
• Conduct outreach and networking to maintain and develop a strong base of client involvement and community support.
• Make regular contact with parents, families and children.
• Provide a variety of groups and workshops designed to increase knowledge and self-help in the client group.
• Participate in assigned meetings.

• Complete and maintain all service documentation and statistics as required.
• Execute duties in accordance with Fighting Back Partnership and grantor’s guidelines.
• Understand and follow all Fighting Back Partnership safety policies, procedures and rules.

Skills and Qualifications:

Require outstanding people skills, a professional attitude; BA or BS degree from an accredited college or university or minimum of 3 years equivalent experience in the areas of serving families, child development, community outreach, and public health, along with the following skills:

• Demonstration of commitment to and competence for dealing with children and families.
• Demonstrated awareness of, sensitivity to, and competence in dealing with cultural and socioeconomic diversity of client population.
• Knowledge of trauma-informed care and practices preferred.
• Knowledge of and skills in working with community-based and public agencies.
• Excellent communication skills, including the ability to communicate at all levels: oral and written with a specific emphasis on interpersonal communications with clients, community members, colleagues, and supervisors.
• Knowledge of community resources, preferred.
• Proven problem-solving skills.
• Valid California driver’s license, a satisfactory driving record and personal vehicle insurance coverage. Must have reliable transportation.

OTHER: Will honor and abide by rules of confidentiality. Must be able to meet physical requirements of the position. These may include long periods of sitting at a desk and/or computer station; using a telephone; lifting items that weight up to 20 pounds; reaching, bending, and stooping within the daily routine of tasks and activities.
