POSITION SUMMARY
This position is responsible for the operational success of the Medical Integrated Community Health Care Program. The Community HealthCare Coordinator is responsible for managing day-to-day program operations, ensuring contract requirements are achieved, complying with regulations, supervision of staff, policies and procedures, maintaining positive customer relations, implementing best practices and achieving program goals and outcomes. This position also provides direct service including outreach, community presentations, and distribution of information. The Community HealthCare Coordinator will commute to Children's Primary Medical Group (CPCMG) in Oceanside, Vista, Escondido, and Euclid Ave (SouthEast).
ESSENTIAL FUNCTIONS
RESPONSIBILITIES
- Responsible for ensuring the safety, security and well-being of clients involved in the programs
- Ensure that Community HealthCare Navigators accurately enter all services into secure database within expected timelines
- Provide support to team while demonstrating appropriate and professional boundaries
- Attend team trainings including HIPAA, secure online case management system, data sharing and management, reporting, interpretation services and other administrative support
- Participate in community events such as schools, food drives, churches/mosques and other faith-based organizations to provide health and wellness outreach and support
- Responsible for ensuring the Program is collaborating effectively with the medical clinic and staff including providing consultation and training.
- Responsible for developing, maintaining, and implementing program policies and procedures that are enriched with positive behavior support
- Collaborate with team to ensure program consistency
- Maintain case records for the program and generate reports to meet program deliverables
STAFF SUPERVISION
- Directly supervise Community HealthCare Navigator(s)
- Balance delegation of duties with providing direct support/coaching to teams while maintaining the ultimate accountability for overall performance, results and outcomes
MINIMUM COMPETENCIES
EDUCATION & EXPERIENCE
- Bachelor’s degree in related field required
- Minimum of 2,000 hours working as a Community Health Care Worker x in paid or volunteer position within the last 3 years required
- Minimum 1 year of supervision
- Demonstrated experience in implementation of current best practices in the field
- Demonstrated experience effectively managing budgets
- Demonstrated experience successfully managing contracts
- Experience in analyzing data and creating management level reports
- Experience utilizing EPIC Case Management System preferred
KNOWLEDGE, SKILLS & ABILITY
- Ability to travel frequently throughout San Diego County
- Experience in program management/coordination and evaluation
- Experience working with Community Health Care Workers or Peer Partners preferred
- Ability to effectively manage time schedule including events, meeting timelines and other activities
- Demonstrated ability to outreach to community, facilitate groups, and develop new programs suitable for youth
- Fluency in Spanish or other language highly desirable
- Excellent verbal and written communication skills tailored to all community and professional levels
- Effective organizational skills and ability to prioritize multiple tasks
- Good computer skills
- Experience in case management highly desirable
ADDITIONAL EMPLOYMENT REQUIREMENTS
- Must be 18 years of age or older
- Must receive Fingerprint Clearance upon hire (paid by Harmonium)
- Must pass Drug Test upon hire (paid by Harmonium)
- Must provide recorded proof of the following items upon hire:
- Annual Flu shot
- TB test results no older than 1 year
- TDaP vaccination within the last 10 years
- 3 Hepatitis B vaccinations or proof of immunity
- 2 Measles, Mumps, Rubella (MMR) vaccinations or proof of immunity for each
- 2 Varicella (Chicken Pox) vaccinations or proof of immunity
- Primary COVID-19 series plus 1 booster
- Must have reliable transportation valid CA Driver’s license and maintain proof of current automobile insurance
