POSITION PROFILE:

Community Health Navigators are trained community-based health workers (CHW) who serve as a bridge between communities and health care systems. CHW provide case management support to community members through intensive coordination of health and health-related services. CHW coordinate client care between multiple providers to address social determinants of health. The role of CHW involves conducting comprehensive health assessments, guiding individuals in developing personal healthcare plans and goals, and connecting individuals to resources and support in the community, all with the goal of improving overall health outcomes. Main duties include assisting members with care coordination such as health care navigation, connection to preventative care services, health education and management, screening and assessments, community resources, referral management, addressing barriers to physical and mental health care, and providing general health and social advocacy. Service sites will be Children’s Primary Care Medical Group (CPCMG) locations (Chula Vista, El Cajon, and Southeastern San Diego).

ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:

  • Serve as a community-based advocate and resource for health plan members, using knowledge of the community and resources available to engage and assist clients in managing their healthcare and social needs.
  • Engage clients with motivational interviewing and trauma-informed care practices to conduct initial intakes, confirm program eligibility, provide ongoing outreach, and set up meetings in the most easily accessible setting for members, such as in-person appointments at their homes, SAY offices or other community locations, telephonic and/or virtual settings.
  • Work to promote collaboration and leadership in the target communities by engaging members, care team personnel, managed health plan organizations, and community agencies in the development of unified goals, objectives, and strategies.
  • Contact eligible members to assist in development, implementation, evaluation, and revision of Comprehensive Assessments, Social Determinants of Health Assessments, and individualized Care Plans and objectives, which may include accessing social services such as community-based resources for housing, food, employment, etc.
  • Provide non-clinical paraprofessional duties, to include: empowering clients by aiding to navigate and maximize their health plan benefits, conducting health promotion and self-management skills training, monitoring treatment adherence (including medication), scheduling appointments with providers, arranging transportation for healthcare visits, getting prescriptions filled, and following up with clients on missed appointments.
  • Advocate on behalf of clients with health care professionals, work with medical facility/hospital staff to ensure smooth and well supported transitions in care, guide clients to maintain Medi-Cal eligibility, and other financial resources as appropriate.
  • Maintain ongoing communication with program staff, service providers, provide timely and accurate documentation of service delivery, status, progress of clients, and sustain required Medi-Cal productivity standards.
  • Establish new and maintain existing linkages with referral sources, conduct professional outreach to promote applicable agency services, and serve as liaison between SAY and community to include accurate representation of the agency and maintain positive working relationships with the general public, clients, funding sources, referral sources, etc.

QUALIFICATIONS (Education, Experience and Certifications):

  • High School Diploma or Equivalent required.
  • Experience working as a CHW in paid or volunteer position within the last 3 years (minimum of 1 year preferred).
  • Understanding and connection with the community and population to be served, including awareness of available resources, community conditions, barriers, and assets.
  • Experience providing case management/case work services to underserved or special needs populations with varied and complex health, behavioral health, economic and educational circumstances.
  • Experience working in a health care or clinical setting preferred.
  • Experience utilizing EPIC Case Management System preferred.
  • Must complete minimum of 6 hours of additional training annually.
  • Must have access to reliable transportation and be willing to use it for job-related tasks.
  • Fully vaccinated against COVID-19 at time of hire

  • Ability to demonstrate knowledge of trauma-informed care and practices
  • Expertise of relevant resources within the community.
  • Experience working with minority populations and broad knowledge of diverse cultures and sensitivity in dealing with diversity issues.
  • Ability to work independently, take significant initiative, manage multiple demands, and prioritize when needed.
  • Possess and maintain positive and supportive relationships with coworkers, strong communication, relationship-building skills, and ability to collaborate and work in a team-oriented atmosphere.
  • Maintain accurate knowledge about the agency, its programs and service.
  • Accurately represent the agency to and preserve positive working relationships with the general public, clients, funding sources, government bodies, etc.
  • Communicate positive and negative feedback from the community through your supervisor to management and utilize appropriate opportunities to purse positive public relations for the agency.
