Job description
Advocates for Independence is looking for Case Managers who are interested in working with the elderly and disabled populations. Case Managers are assigned an average caseload of between10 to 22 patients. These patients all reside in community settings; specific duties listed below.
The position requires monthly contact with each client. Staff set their own work schedules and have the option of working from their homes. All technology and equipment needed will be issued by the agency. Those hired will receive paid training. Part-time staff are eligible for Dental, Vision, Life Insurance and Short-Term Disabled paid for by the agency.
Case manager duties include assessing, service coordination, advocacy, safety planning, monitoring, and the timely completion of required documentation and forms. You must be licensed in the State of Utah as a Social Services Worker or higher.
Responsibilities
- Coordinate and provide monitoring that is safe, timely, effective, efficient, equitable, and person-centered.
- Handle case assignments, draft service plans, review case progress and implement interventions as needed.
- Help clients achieve wellness, freedom of choice and independence.
- Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
- Develop effective working relations and cooperate with medical team throughout the entire case management process
- Follow the policies and procedures required by this agency and state agency partners.
- Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
- Record cases information, complete accurately all necessary forms and produce statistical reports
- Promote quality and cost-effective interventions and outcomes
- Assess and assist with client’s meeting their personal goal.
- Adhere to professional standards as outlined by protocols, rules and regulations
Skills
- Proven working experience in case management, including as a nurse, medical, mental health care manager or a related job
- Excellent knowledge of case management principles, healthcare management and reimbursement
- Previous experience with psychological aspects of care
- Effective communication skills
- Excellent organizational and time management skills
- Familiarity with professional and technical emerging knowledge
- Problem solving skills and ability to multi-task
- Compassionate with teamwork skills
- Current license, certification or registration
- BS degree in related healthcare field
Starting pay is $22.50 -$25.00 per hour depending on experience. Paid training, mileage reimbursement, excellent benefits for full and part-time staff, flexible schedules and the option to work from home.
