At Sunrise, our Care Manager is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living and reminiscence neighborhoods.
Responsibilities
Responsibilities:
- Build meaningful relationships with seniors that reside in memory care and their families as you provide assistance with activities of daily living, attend to individual care needs and get to know their unique preferences and personalities
- Committed to serving our residents and guests through our Principles of Services
- Assist residents in life skills and other life enriching activities as indicated on their individual profile
- Blend a variety of multi-sensory experiences into the resident's day
- Participate in the development of the Individualized Service Plans (ISP) and monthly updates
- Responsible for all or assigned group of residents during the shift, being aware of residents' location and needs at all times.
- Partners with community team to ensure community is in compliance with OSHA requirements and adherence to safety rules and regulations.
- Responds to the dining needs of the residents and guests while maintaining Sunrise hospitality and service standards
- Invite, Encourage, Assist (IEA) residents in life skills and other life enriching activities as indicated on ISP and demographic profile
- Inform Wellness department of any changes in condition and recommended adjustments in the level of care and service
Qualifications:
- Must have minimum 1-2 years experience working in senior living community or with seniors.
- Experience working in Memory Care, a plus.
- CNA certification a plus, certification must be current and continously maintained per state regulations
- College preferred, High School diploma/GED accepted and may be required per state regulations
- Must possess excellent written and verbal skills for effective communication and a level of understanding
- Excellent customer service skills
- Abiity to work with diverse groups of people
- Ability to be flexible in performing various duties in service to the residents in both Assisted Living and Memory Care when asked to do so.
- Ability to make appropriate decisions and act in the resident’s best interest
- Possess great organizational and time management skills
- Demonstrate good judgment, problem solving and decision making skills
