Behavioral Health Case Manager/Community Health Worker


Location: 1233 Edgewater Street NW, Salem OR 97304

Job Status: Full-time, Monday – Friday, 8am – 5pm

Sign-on Bonus: $1000 (Internal candidates that are currently employed here at NWHS are not eligible for a sign-on bonus.)

Employee Referral: $500 (Referral bonus’ expire at the close of this job posting.)

OUR ROLE:

This position is part of the care coordination team that works alongside a multi-disciplinary team of Community Health Workers, Mental Health, and Medical Professionals to address barriers to positive health outcomes. As a Behavioral Health Case Manager/Community Health Worker you will provide resources, coordination, support, and wrap around service for clients of NWHS. Duties include basic case management, resource development, and functioning as the lesion between patients, community and NWHS.


SPECIFIC DUTIES:

  • Build rapport, practice active listening, show empathy and compassion, demonstrate cultural awareness and humility, and be honest and transparent.
  • Make referrals, manage calls, set treatment plan goals, and facilitate group settings, with the goal of empowering clients to set and work towards their own goals. Interventions will include, but not be limited to, connecting clients to community resources, building self-advocacy skills, determining readiness for change, navigating ambiguity, installing confidence, optimism, and validation.
  • Assist clients in attending medical and mental health appointments, ensuring that they receive the care they need to maintain their health and well-being.
  • Network with community-based organizations and other resources to promote client services, care, education, and advocacy.
  • Document referrals, tasks, and interventions in NextGen. Maintain up-to-date, timely, accurate, and complete care coordination notes within the electronic health record and follow procedure for appropriate follow-up. Be organized and utilize a systematic approach to information management. Collects data and tracks interventions and outcomes to support strategic planning processes. Submit appropriate billing codes.
  • Carry a case load and use judgment to determine the frequency, quantity, and timing of follow-up interventions. Assist clients in developing and implementing a care plan tailored to individual needs and concerns, with the goal of improving basic functioning in daily and community living. Provide closed-loop communication back to referral sources as needed.
  • Process referrals for Mental Health Counseling, triaging as needed.

QUALIFICATIONS:

  • Bachelor's degree in a behavioral sciences field; or a combination of three years' relevant work, education, or experience for QMHA eligibility.
  • Complete Community Health Worker certification within six month of hire and maintain current.
  • Familiarity with mental health environment and workflows.
  • Knowledge of community resources and support systems.
  • Ability to work independently as well as collaborate with a multi-disciplinary team.
  • Oregon Driver's License and proof of auto insurance (this position requires driving, and a motor vehicle report is conducted prior to hiring)
  • Bilingual English/Spanish preferred.
