Job description

Assess youth to provide case management to all youth assigned to caseload.

develop Individual service plans, and refer clients to services and programs, particularly asset development programs;

Coordinate and conduct monthly case conferences with each client and their external case managers.

Develop Individual Service Plans with youth to ensure steps are identified and supported regarding all housing search needs;

Assist youth in completing housing applications and obtaining housing interviews;

Remain up-to-date on housing regulations and programs relevant to program participants;

Ensure that all services are documented (e.g. face to face, phone, and collateral contacts) in a timely and thorough manner;

Document client outcomes and enter client data into HMIS database;

Coordinate And implement monthly life skills sessions;

Provide up to six months of After Care services to clients who successfully exit the program.

Participate in the client interview process

Accompany clients to appointments as needed;

Assist with the client intake process

Represent the organization in local community meetings.

Other duties as assigned by the supervisor.

Job Type: Full-time

Pay: $40,000.00 - $45,000.00 per year
