Earn – $24 to $27.31/Hourly
Position Details
The Intensive Transition Services (ITS) program offers support to adults who have both a serious mental illness and a developmental disability in a multidisciplinary team setting. Participants receive clinical therapeutic interventions, care coordination and help dealing with other issues in their daily lives.
- Provide recovery based, person centered services
- Develop comprehensive goal plans in conjunction with individuals and family members.
- Provide Service coordination, including coordinating and monitoring the activities of the individual's treatment team.
- Develop, write, implement, evaluate and revise, as necessary, individual's treatment plans in collaboration with the Treatment Team.
- Provide individual and group supportive interventions and symptom management ensuring that immediate changes in the treatment plan are made, as necessary.
- Act as a liaison and consult with community agencies and families to maintain coordination in the treatment process.
- Educate and support individual's families and advocate for individual's rights and preferences.
- Conduct comprehensive assessments of the following areas:
Family structure and relationships
- Psychiatric history
- Substance Abuse
- Any history of drug and/or alcohol use
- Social development and functioning
- Daily living activities
- Document individual's progress while maintaining a permanent record of individual's activity in accordance with policies and regulations.
- Provide treatment, rehabilitation and support services in conjunction with other team members and with direction from staff with specialty training.
- Provide on call crisis intervention as required.
- Participate in daily organizational staff meetings and treatment planning meetings.
- Perform other related duties as required or assigned.
- Participate in cross training with other specialists as deemed necessary by supervisor.
Structuring Time and Employment
- Provide individual vocational-supportive counseling to enable individuals to identify vocational strengths and areas to improve upon as well as establish vocational or career goals and plans to reach them.
- Plan, structure and prompt social and leisure time activities on evenings, weekends, and holidays.
- Organize and lead individual and group social activities to increase social experience and provide opportunities to practice social skills and receive feedback and support.
- Provide mentoring, problem solving and support on and off job site.
- Perform work-related support services, such as wake-up calls, transportation and securing of appropriate clothing.
Activities of Daily Living Services
- Provide ongoing assessment, problem-solving, side-by-side services, skill training, supervision and environmental adaptations to assist individual activities of daily living.
- Assist individuals through education and promotion of good hygiene. (grooming, bathing, brushing teeth, etc.)
- Assist individuals in the procurement of safe and affordable housing and related necessities (utilities, furniture, etc.)
- Support individuals in the performance of household activities (cleaning, organizing, laundry, etc.)
- Provide nutrition education and assistance with meal planning, grocery shopping and meal preparation.
- Ensure that individuals have adequate financial support and teach and promote money management skills.
- Assist individuals in accessing reliable transportation.
Social and Interpersonal Relationships and Leisure Time
- Provide individual support therapy, social skill development and assertiveness training to increase individual's social and interpersonal activities in community settings.
- Provide side-by-side support, coaching and encouragement to individuals to help facilitate social interactions.
- Support individuals in planning and participating in leisure time activities.
Support
- Provide practical help and supports, advocacy, coordination, side by side individualized support, problem solving, direct assistance, training and supervision to help individuals obtain the necessities of daily living.
Other
- Attend daily team meetings as assigned
- Attend all other meetings as assigned
- On call duties as assigned
- Follow all Merakey policies including but not limited to standards of conduct, confidentiality requirements, standards of professional behavior
- Document in accordance with Merakey policies
- Complete required productivity, mileage and other reports in a timely fashion as required
- Other duties as assigned

Requirements:The ideal candidate will possess the following qualifications:
- Bachelors degree in Social Work, Psychology, or another human service related fields
- One to two years of experience in the behavioral health field working with individuals with development disabilities.
- Preferred:One year of experience in the provision of case coordination and case management services
