Under the direction of the Director of Behavioral Health and as a part of a multidisciplinary team, Therapists provide direct services for individual youth, couples, family, and group therapeutic services to patients. Therapy services may include crisis intervention, assessments, treatment plans, and discharge planning. Therapists coach patients to manage ongoing health conditions and provide therapy to help patients make positive behavior changes.
Responsibilities, Deliverables, Outcomes and Expectations
- Work well with minimal supervision
- Administer, interpret, and document results of screening and assessment tools
- Provide diagnostic impression.
- In cooperation with patient, produce and document individual service plan to include determination of appropriate level of care, treatment interventions and estimated length of stay (number of sessions) to realize treatment goals.
- Provide individual and group counseling to patient and, as necessary, to patient’s family and significant others and document in patients file.
- Be immediately available to the health center staff during working hours in the event of a psychiatric emergency.
- Provide consultation and training to other Center staff employees as appropriate as well as provide consultation and training to community organizations as appropriate.
- Complete all charting as needed and required by Valley View Health Center policies, including assessments, progress notes, and billing.
- Participate in case management activities related to individual clients.
- Participate in any reviews and/or inspections necessary for health center licensure/certification.
- Comply with Safety Policies and Procedures of CAC and VVHC.
- Attend meetings, conferences, and training sessions related to the duties of this position and disseminate acquired information to appropriate individuals inside and outside the agency.
- Responsible, along with other staff, for keeping the health center, office areas, kitchen and supply areas clean and orderly.
- Educate patients regarding their medication regimen and compliance with behavioral health activities.
- Contact patients in regards to scheduling follow-up appointments with providers as necessary as well as contacting patients and scheduling well checks as they are requested by providers.
- Remain calm in times of crisis.
- Track and document patient follow-ups and contact patients via phone or mail to send reminders to schedule appointments.
- Provide individual, couples and/or family consultations
- On an as needed basis, assist nursing staff with preparing patients for their appointments and discharging patients.
- Schedule both incoming and outgoing referrals and continue to follow up on said referrals.
- Answer and maintain communication with any patients that call the Behavioral Health Line and direct them to the proper resources as necessary.
- Assist with other duties as defined by assigned supervisor when other work is necessary to fulfill the obligations of the Valley View Health Centers and/or CAC.
- Accountable to the assigned supervisor for high standards of accuracy, attention to detail, and timely completion of work.

Must behave and act in ways between and among co-workers that is conducive to team work, cooperation and an overall effort to ensure a pleasant and positive work atmosphere. Must maintain a courteous, professional demeanor with patients and maintain a positive reflection of CAC and the Valley View Health Centers.
Minimum Requirements…
Standards of Conduct and Conflict of Interest self-declaration.
Annual blood borne pathogen training.
Annual HIPPA training.
Current valid driver’s license, proof of automobile insurance, insurable under agency insurance policy, reliable transportation, compliance with VVHC’s Employee Uniform/Attire.
License to provide Behavioral Health services in the state of Ohio (LISW, LPCC, IMFT)
Primary Source verification of education and training.
Current BLS/CPR certification.
Three (3) professional letters of recommendation.
Proof of annual PPD (TB) test.
Proof of Hepatitis B immunity, including a copy of shot record showing the Hep B series.
Proof of most recent influenza vaccine.
Preference given to those with 2 years’ experience; Preference given to individuals with a wide diversity of professional experiences as well as experience in a primary medical care setting.
