Targeted Case Manager

River Edge is needing Targeted Case Managers to work as a part of us becoming a Certified Community Behavioral Health Clinic or CCBHC. CCBHCs are designed to provide whole-person care by integrating physical health with a comprehensive range of mental health and substance use disorder services to vulnerable individuals. CCBHCs are a game-changer and River Edge wants to change the lives of the community we serve to make living better for everyone.

Duties & Responsibilities:

• Provides a broad range of paraprofessional social services to clients' and their families
• Maintains contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on client's overall progress
• Provides short-term case management and referral services to clients with emergency situations
• Participates in treatment team planning, and performs crisis intervention as appropriate
• Assists in dealing with personal and social problems
• Maintains medical records and documentation of daily progress notes in a timely manner
• Refers clients to needed services and resources
• Prepares monthly case reviews of progress toward clients' treatment goals
• Other case management and clerical duties as assigned

Minimum Training & Experience:

• COVID Vaccinated
• Bachelor's degree from an accredited college or university in a social service related field (social work, sociology, psychology, etc.)
• Experience with Microsoft Office Suite preferred. Must also be able to learn other relevance software applications including electronic medical records
• Must have a valid Georgia Driver's License and good driving record
• Must be willing to transport clients in personal vehicle
