Job Description

This is a managerial position which involves overall accountability for the daily coordination and operations of Care Coordinator members provided by Oishei Healthy Kids Health Home. This candidate will report to the Health Home Administrator and will supervise a team of 5-8 Care Managers. This position will provide services to children/adolescents up to the age of 21. This candidate ensures that the Care Coordinators use a holistic approach to care and that goals for all dimensions of health; psychosocial, behavioral, emotional and medical are met. This candidate collaborates with Social Work, all Care Coordinators, Medical Staff, Nursing, Ancillary services to ensure that children/adolescents care is coordinated, with all accompanying documentation and referrals, appropriate community referrals, and completed family/children/adolescents education and linkages. This candidate supervises staff, oversees daily program activities and ensures ongoing staff development. This candidate provides oversight, reviews and updates procedures to assure quality indicators are met and facilitates change when necessary.

Minimum or Preferred Qualifications

BA - Bachelor of Arts Social Work, Human Services or related health care field required. 3 years of experience in providing care to persons with mental illness, chemical dependency, developmental disabilities and/or a chronic medical conditions required. 1 year of experience in a supervisory capacity with Caseworkers, Coordinator Care, Managers or similar Care Management positions preferred. 1 year of experience in Health Home Setting preferred. NYS Drivers License required upon hire. CANS-NY 0-5 Year Old and CANS-NY 6-21 Year Old within 30 days required.
