Step-Down Navigator Opportunity –

This is an excellent opportunity for a masters of social work student or mental health counseling student to gain valuable mental health experience while completing your masters!

The Step-Down Navigator is a vital part of Arbour Hospital’s Outpatient Services team. This is a full time 40 hour per week position.

Responsibilities

• Meet with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment
• Communicate eligibility to key members of senior leadership & associated departments
• Ensure the Step-Down process is maintained to provide continuity of care
• Evaluate trends and establish goals as it pertains to the step-down process
• Ensure the collection and entry of data as it pertains to patient eligibility
• Review & Report on data and processes routinely in Quality/PI meetings
• Consults with the Social Services, Utilization Management and Outpatient on patient eligibility
• Assist in the development of strategies, goals and objectives related to the step-down process

Requirements

• Education: Bachelor’s degree in mental health/behavioral sciences field preferred
• Experience: Minimum of two (2) years of experience in the mental health field. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred.
