Duties & Responsibilities


Perform all duties in alignment with the mission, vision, values, and behavioral expectations of NYC Health + Hospitals
Conducts daily home visits (ACT staff are granted privileges to drive HHC vehicle), and/or telehealth as per protocols, participate in intake screenings to enroll new clients into program, participate in family sessions, visit client at the hospital, coordinate care with outside agencies, as needed; may co-lead groups, facilitate social activities for clients as needed.
Development of psychosocial assessments and social work goals through history obtained from patients, family members/significant others and other sources to whom patient is known
Provision of supportive counseling and psycho-education for patients, family members/significant others.
Advocates for patients, family members, significant others with community resources, i.e. benefit and entitlement programs.
Member of interdisciplinary team(s) responsible for the formulation of treatment plans during the course of treatment and discharge plans for returning/maintaining patients to community living.
Leader/Co-leader of patient group therapy meetings and/or community meetings.
Provision of appropriate care services for patients, i.e. housing, residential/outpatient treatment services, special needs, i.e. homecare services, etc.
Coordinates activities performed by the Caseworker who assists the Social Worker with provision of services to patients, family members/significant others, if applicable.
Provide most of the treatment in the community, or where the client may prefer.
Conducts AOT reporting weekly and service verification each month and as needed for AOT cases assigned.
Completes initial and update assessments in CAIRS reporting when due for assigned clients.
Demonstrates core competencies in designed areas of practice, including the Assertive Community Treatment Core processes, integrated mental health and substance abuse treatment, supported employment, family psycho-education and wellness self-management.
The Vocational Specialist is responsible for completing Initial Vocational Assessment and every 6 months updates.
The Vocational Specialist is responsible for providing vocational counseling and employment search assistance; to assist client with identifying interests and skills.
The Vocational Specialist is responsible to assist clients with job coaching and social skills training and to develop and strengthen relationships with employers and other vocational support agencies.
The Vocational Specialist is responsible for assisting clients identify and pursue vocational recovery-oriented goals and to educate potential employees, if allowed by the client, about mental illness.
Other responsibilities pertinent to treatment and team functionality, including QI Projects to be developed and completed, as clinically indicated and assigned by Team Leader

Minimum Qualifications


Knowledge of human growth and development and an interest in working with the mentally ill population. A Master’s degree in Social Work from an accredited Social Work School. Must be licensed (LMSW) or (LCSW).
