The Paramount at Somers, 2022 Deficiency-free Facility, is seeking a full-time Social Worker for our skilled Nursing Facility located in Somers, NY.

The ideal candidate will have excellent communication skills and prior long-term and/or subacute experience.

JOB REQUIREMENTS:

• Master’s degree in social work preferred

• Long Term Care/ Subacute experience recommended

RESPONSIBILITES INCLUDE:

• Complete a social history and psychosocial assessment for each resident that identifies social, emotional, and psychological needs.
• Participate in the development of a written, interdisciplinary plan of care for each resident that identifies the psychosocial needs/issues of the resident, the goals to be accomplished for those needs/issues, and the appropriate social worker interventions. Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident.
• Provide clinical interventions to address events that occur during the resident’s stay in the facility.
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Coordinate the resident discharge planning process and make referrals for appropriate home care services prior to the resident’s return to the community.
