Job Summary
Gwinnett, Newton, and Rockdale County Health Departments is recruiting for a HIV Health Navigator to work alongside a multi-disciplinary HIV prevention team and community partners specializing in HIV Prevention among the substance misuse population. Candidate will assist partners with the integration of HIV prevention strategies, including HIV opt out testing, linkage to care, PrEP eligibility determination and linkage, into their programmatic functionality; plan, coordinate and implement HIV Prevention outreach and testing events in the community; communicate HIV Prevention and substance misuse education and resources to the public through presentations and health fairs; provide HIV prevention education and support to health care providers; perform offsite, rapid HIV testing for community partners; assist community partners with client linkage to health center services; and perform other duties as assigned to meet programmatic and/or organizational needs. Position is grant funded under a 5 year SAMHSA grant.
Qualifications
Minimum
High school diploma or GED AND One year of experience in a social services related position OR Two years at an accredited college or university with at least 15 credit hours in a social services related field.
Preferred
Experience working with Substance Misuse and/or HIV Prevention programs or patient populations. Experience with public speaking or providing education to individuals or groups. 1+ year of customer service experience. Bilingual a plus.
