Santa Anita Convalescent Hospital is hiring a full time Social Service Designee (Please note this is not a Bachelors/or Masters position)

Must possess a Social Service Designee Certificate

This is a full time position with the hours of 8:30 AM-5:00 PM (schedule to include weekends)

The primary purpose to your position is to assist in the planning, developing, organizing, implementing and evaluating of the Social Services Department in accordance with current existing federal, state and local standards, as well as our established policies and procedures, to ensure that medically related emotional and social needs of the resident are met/maintained on an individual basis.

Responsibilities:

  • Understands and follows all policies and procedures.
  • Receives and relays messages.
  • May be asked to log calls and perform follow-up.
  • May supply information to callers and record messages.
  • May perform additional clerical duties.
  • Works with staff and patients and family, to ensure satisfaction in such areas as quality, service and problem resolution.
  • Complies with all Company HIPAA policies and procedures.
  • Complies with all company safety and risk management policies and procedures.
  • Applies all applicable OSHA and related local safety requirements to all assigned work.
  • Performs all work in accordance with established safety procedures.
  • Perform other duties and responsibilities as assigned.

Physical Requirements:
  • Close vision, distance vision, peripheral vision, depth perception and the ability to adjust focus, with or without corrective lenses.
  • Significant walking or other means of mobility.
  • Ability to work in a standing position for long periods of time (up to 8 hours).
  • Ability to reach, bend, stoop, push and/or pull, and frequently lift up to 25 pounds and occasionally lift/move 35 pounds.

Other Related Responsibilities Includes:

  • Possibly Manage a caseload of 65-70 residents
  • Complete Psycho-Social Assessments, quarterly and follow up on resident ancillary needs
  • Communicate with Core team and assist conservators/ counties with discharge planning
  • Assist residents with daily concerns and requests such as clothing needs, ancillary needs,theft and loss
  • Fax packets to appropriate individuals for discharge planning
  • Attend admission meetings with new residents
  • Assist residents to vote if they choose to vote during elections

Qualifications / Essential Skills Include:

  • Must be able to read, write, speak and understand English
  • Must be able to follow written and oral instructions
  • Must have good computer skills Must uphold and maintain professional behavior at all times.
  • Must follow facility policy as it relates to patient care, treatment, intervention and documentation.
  • Strong problem solving and critical thinking skills.
  • Dependable and Good work ethics.
  • Ability to interact with residents in the milieu and model appropriate social and professional behavior.
  • Treat all residents with dignity and respect. Experience in mental health and substance abuse populations preferred but not required.
  • Ability to work with patients in a manner that is productive and coincides with patient treatment goals.
  • Must have at least 1 year of Social Service Designee experience in a Skilled Nursing Facility
  • Bilingual in Spanish (preferred)
Experience/Knowledge:
  • High School diploma, GED or equivalent experience.
  • Certification: Must possess a Social Service Designee Certificate
