RN Field Supervisor for Home Care Agency

Part Time/Per Diem Positions Available

Recco Home Care Service, a New York State Licensed Home Care Agency and CDPAS Fiscal Intermediary, is a leader in the home health care industry servicing families in Suffolk, Nassau, Queens, Brooklyn, Bronx and Westchester County for over 50 years.

We are looking for reliable, flexible and compassionate Registered Nurses (RN), who enjoy working with the elderly and disabled population and want to make a difference in the lives of others. Position requires traveling to the client's home to perform nursing assessment, develop Plan of Care for home care services and supervise/evaluate the caregiver in the home. The Registered Nurse(s) will also make periodic nursing assessments to review and revise the plan of care as necessary.

The RNs should have knowledge of NYS DOH regulations for Home Health Care and to be familiar with New York LHCSA procedures.

Registered Nurse Position Qualifications:

• Experience in Home Care
• Ability to work independently and effectively maintain an active caseload
• Ability to communicate effectively and maintain accurate client records
• Motivated and outgoing personality
• Valid New York State RN License
• Spanish/Bilingual a plus

Job Functions / Responsibilities:

• Complete an initial home health assessment of client and develop an individualized plan of care.
• Maintain an active caseload of clients
• Conduct regular home visits for each client timely as required by individual contract.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Perform skill assessments on HHAs/PCAs in the office and in the home, as needed.
