Job description

• Visiting with patients and family members frequently to monitor their physical, emotional, and religious needs
• Evaluating patients’ social and health histories to design treatment plans that are tailored to their specific needs
• coordinating diagnostic and support plans for each patient with specialists, nurses, and other health professionals
• Services such as caregivers, nursing care, and hospice care are coordinated and arranged.
• Keeping track of patient treatment plans, physician instructions, medication management, and other pertinent information concerning a patient’s health.
• To fulfill the requirements of patients, coordinate care with other health personnel and social service agencies.
• Offering guidance and assistance to patients and families at tough times
• Lead a hospice team of care providers

Pay: $55.00 - $60.00 per hour
