Care Managers are essential employees who positively affect both patient and organizational outcomes. The care management department is comprised of highly skilled and compassionate professionals who provide superior care to a diverse patient population. Responsibilities:

• Facilitate the patient’s hospitalization from pre-admission through discharge.
• Coordinate with Physicians, Nurses, Social Workers, and other healthcare providers to expedite quality care.
• Apply clinical expertise to resource utilization and discharge planning.
• Communicate with administration and the healthcare team as needed.

Qualifications:

• An ADN is required, and a BSN is preferred.
• 3+ years of clinical experience in an acute care setting.
• Prior care management experience is required.

Additional info:

• Full-time, permanent position
• Hours: 8a-430p
• Pay: $126,000-$167,000, depending on experience + bonuses.
