  • Soin Medical Center has been serving residents of Greene, Western Clark, Eastern Montgomery, and Miami counties since 2012.
  • Conveniently located off I-675 in Beavercreek.
  • Provides a variety of health care services and is home to 4 accredited Centers of Excellence including hernia and robotic surgery.
  • Soin is currently licensed for 125 beds which will increase after the expansion completion in the Fall of this year.

Responsibilities & Requirements:
  • The RN Care Manager may be assigned to a variety of areas such as the Emergency Room or as a member of a Care Coordination team.
  • Specific job functions will vary based on assigned area but would include items such as: admission status assignment, core measures identification and management, procedure indications and scheduling verification, present on admission conditions and comprehensive discharge planning.
  • The RN Care Manager evaluates cases to determine the utilization and quality aspects of admission as it occurs in the acute care setting.
  • The RN Care Manager facilitates modifications to clinical documentation through extensive concurrent interaction with physicians, nursing and ancillary staff to support utilization, evidenced based practice guidelines and appropriate documentation of medical necessity to supports timely and accurate hospital initiatives and reporting physician and hospital outcomes.
  • Weekend and holiday shift rotation required.

Preferred Qualifications:
Minimum Work Experience:
· Charge Nurse, Nurse Manager, Case Manager, Care Coordinator, Care Manager experience preferred · Five years clinical experience, Critical care or ED experience preferred with two years’ experience in case management, clinical outcomes and/or as a clinical documentation specialist preferred

Required Licenses, Certifications and Registrations
(Include licenses, certifications and registrations required by the regulatory or governing body of that discipline.):
· Registered Nurse with a current license to practice in Ohio · Certification from an accredited organization pertaining to Clinical Quality, Case Management, Care Coordination, or transitional care preferred

Required Skills:
· Works well in a multidisciplinary setting and independently to facilitate patient comprehensive needs. · Good verbal and written communication skills to maintain good rapport with physicians, hospital administration, nurses and other health team members. · Ability to prioritize and work with multiple activities essential. · Knowledge of and demonstrated efficiency and accuracy with computer skills related to Microsoft applications, Midas and hospital specific programs required · Familiarity with quality improvement principles and their application preferred. · Knowledge of federal, state, Hospital Facilities Accreditation Program (HFAP) and quality improvement organization regulations and their application needed. · Ability to accept responsibility, analyzes problems, evaluate reports, and make decisions and recommendations required. · Ability to demonstrate initiative and good judgment in a self-directed role. · Ability to use and to interpret variance and statistical data. · Must be flexible and able to manage job functions despite shifting priorities
