Pay: $38.62 hourly
Location: Remote/Member facing Montgomery, Greene, or Clark counties OH
Schedule: Mon-Fri 8a-5p
Job Description/Requirements: TheRegistered Nurse Case Manager is responsible for making in home or telephonic visits within their population zip code; they manage all day to day needs of members and ensure Health, Safety, and Welfare of all members within their caseload. The Registered Nurse Case Manager will be responsible for completing in person visits with members for acuity contact schedule, Post Hospital visits, and/or LTC request visits, health safety welfare visits, etc.
Experience/Education (MUST be included on resume):
- Active Registered Nurse license for Ohio
- 2+ years of clinical nursing experience in a clinical, acute care, or community setting
- 1+ years of case management or utilization experience in a managed care setting
- Waiver knowledge preferred but not required
