As necessary the RN Case Manager may work in other nursing roles so departmental activities are covered on a daily basis.

Job Overview::
This position functions within Sitka Primary Care to assist patients and medical staff within the ambulatory care setting by providing case management services to an identified patient panel.

The RN Case Manager is assigned to a medical provider and assumes nursing case management activities for that providers’ panel of patients. The RN Case Manager assists patients with their short and long term medical needs by collaboratively setting medical goals. As necessary the RN Case Manager may work in other nursing roles so departmental activities are covered on a daily basis.
Salary: $41.83-$48.61 per hour/DOE
- Collaborates with the medical provider and patient to establish individualized medical and nursing goals. As a point of contact for patients, the Case Manager is expected to use problem-solving and critical thinking skills in meeting the needs of the patient and the provider. It is also expected patient needs are addressed in a prompt and efficient manner (ie: phone calls returned within 24 hours, cross coverage between case managers is developed and followed). Organizes and attends patient care conferences.
- Performs staff nursing duties in the various nursing roles in the clinic. Duties include assessment, documentation, education and administration of appropriate medications and/or therapies in a responsible, respectful, caring manner to promote the most effective patient relationships.
- Works closely with Referral Care Coordinators to facilitate patient referrals within SEARHC, IHS, or to outside agencies as requested by the Provider. Serves as the primary clinical contact regarding patient care. Proactively identifies delays and obstacles to care and works to remedy individual cases and improve the process.
- Maintains annual competencies including safety and infection control requirements. Performs duties as assigned in a responsible, respectful and caring manner to promote effective nursing staff relationships. Maintains a clean and safe working environment including adequate supplies and upkeep of unit and equipment, reporting any defective or missing equipment and safety hazards. Adheres to infection control policy and procedures.
License and Certifications:
- A valid, current, full, and unrestricted RN license in the State of Alaska OR licensed in another State and obtain Alaska license within 6 months of hire
- Basic Life Support certification
- National certification in Primary Care, Ambulatory Care or other pertinent certification, preferred
Experience:
- 3 to 5 years general nursing experience with a minimum of 1 year in the ambulatory care setting
OR
- 2 years general nursing experience with a minimum of 1 year of employment as a clinical nurse at a SEARHC outpatient clinic.

Knowledge of:
- Nursing process and the ability to apply this in the working environment
- Customer service principles
- Safety and infection control principles
Skilled in:
- Completing assessments, anticipation of needs, and specimen collection
- Use of equipment such as computers and medical equipment
- Effective oral and written communications skills
Ability to:
- Multi-task and work independently in fast paced environment
- Maintain compliance with annual competencies
- Problem-solve and use conflict resolution skills to develop and maintain good working relations with other staff and departments
- Demonstrated prior experience in leadership roles using leadership skills
