SLM Services is currently seeking a Registered Nurse in Chicago, Illinois and surrounding areas for a part time position completing Virtual, Telephonic and Face to Face long term care assessments.

Skills/Requirements

- Licensed in Illinois as a Registered Nurse with no restrictions

- OIG/SAM/Criminal Background/Nursys Verification

- Ability to assess an individual's level of ADL and IADL dependency by observing them mimicking their activities of daily living, Ability to communicate with patients by asking various questions regarding their current health conditions, complete an cognitive assessment, Monitor blood pressure, body weight and height

- Work Experience providing care to the geriatric patients

- Ability to communicate with the practice manager using electronic devices (telephone call, email, and text message)

- Own a personal laptop/tablet/ desktop computer with printer and scanner

- Ability to scan and upload documents to return to agency using fax or email

- Must have a valid driver license and car insurance

Job Types: Contract, PRN

Pay: Up to $50.00 per hour
