Responsible for quality improvement activities related to the development, implementation, evaluation and ongoing success of the Ambulatory Quality Program. Partners with medical practices to implement quality improvement initiatives . I nvolvement in team based care programs for chronic disease management, including patient outreach and a component of direct patient care.
Analyzes quality performance of Loyola Medicine employed primary care and specialty practices, and reviews patient safety events across the ambulatory environment related to performance improvement opportunities. Educates clinic staff, providers and other key stakeholders about the quality measures and collaborates with practices to develop systematic processes and best practices to ensure high quality performance. Conducts regular onsite practice visits and serves as a subject matter expert on quality requirements and performance measurement.
Coordinates use of EPIC tools, including reporting workbench and quality dashboards; provides support and education for front line staff on institutional MIPS workflows. Participates in ambulatory quality and patient safety rounds and committees. Reviews ambulatory quality reports with a focus on system-wide initiatives to improve quality, patient safety and reimbursement. Develops instructional materials for staff to standardize patient care and documentation patterns throughout the ambulatory enterprise.
Participates in root cause analysis as a QI team member to identify barriers to quality performance. Views quality thorough a system lens and researches and recommends evidence-based approaches to embedding quality processes into standard care processes. Promotes and supports healthcare outcomes in accordance with the policies, procedures and mission statement of the Ambulatory Quality Program.
